He is using Nova Launcher Prime for his Home screen, but you can do it without the Prime. He used Light X Icon Pack for his App drawer. The Home screen consists KWGT widgets, the top one is from Glow KWGT (widget 39), the bottom ones is from Flawless KWGT (widget 56 & widget 96)
You can also do this with other launchers.
Note: KWGT widget packs like these requires KWGT pro.
